Disclaimer
I, whose name appears on the above hereof, in consideration of and as a condition of acceptance of my entry into the Laguna Phuket Triatlon in Phuket or associated event, for myself, my heirs, executors and administrators hereby waive all and any claim, right of cause of action which I or they might otherwise have for, or arising out of loss of life or injury, or damage of any description whatsoever, which I may suffer or sustain in the course of or consequent upon my entry or participation in the said race and/or associated training sessions. I will abide by the race rules governing these events. This waiver, release discharge shall be and operate separately in favor of all persons, corporations, and bodies involved or otherwise engaged in the promoting of the event and the service agents, representative and officers of any of them. I attest that I am physically fit and have sufficiently trained for completion of the entered race. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and /or illness during the race. I give permission for the free use of my name, voice or picture in any broadcast, telecast, advertising promotion or other account of this event.
